Let’s get started! Over on the right-hand side of this page, you will see two documents to download. One is the Initial Intake Form and the other is the Contract Agreement. Please fill these out and return them to me.

For payment, I ask for half of my fee upon signing the agreement, and then pay the remaining half two-weeks before your due date (is this correct?). I accept PayPal, cash, or check made out to Tiffani’s Doula Services. A PayPal link is below. (You can mail these to me or give them to me when we meet in person??)
